2. Services Description
TOFAD Wellness Clinic provides virtual primary care, chronic disease management, preventive wellness, and related healthcare services via telehealth. Our services are currently available to patients located in Illinois, Indiana, and Wisconsin.
3. Eligibility
You must be physically located in Illinois, Indiana, or Wisconsin at the time of your virtual visit. You must be at least 18 years of age, or have a parent or legal guardian consent to services on your behalf.
4. Not Emergency Services
Our telehealth services are not intended for medical emergencies. If you are experiencing a medical emergency, please call 911 or go to your nearest emergency room immediately.
5. Appointment and Cancellation
- Appointments can be scheduled through our website booking form or by contacting us directly.
- We request at least 24 hours' notice for cancellations or rescheduling.
- Repeated no-shows may result in a fee or dismissal from the practice.
6. Payment and Insurance
We accept Medicare, Railroad Medicare, IL Medicaid, Aetna, Cigna, Humana, Anthem BCBS of Illinois, UnitedHealthcare, Multiplan/PHCS, Tricare, Veterans Affairs, Health Alliance Medical Plans, and Molina Healthcare. Copays and deductibles apply per your insurance plan. Self-pay options are also available.
7. Privacy
Your use of our services is also governed by our Privacy Policy. We comply with all HIPAA regulations regarding the handling of protected health information.
8. Limitation of Liability
While we strive to provide the highest quality care, the use of telehealth has inherent limitations. We are not liable for any delays, failures, or interruptions in service due to technology issues beyond our control.
9. Intellectual Property
All content on this website — including text, graphics, logos, and design — is the property of TOFAD Wellness Clinic and is protected by applicable intellectual property laws.
10. Changes to Terms
We reserve the right to modify these Terms of Service at any time. Continued use of our services after changes constitutes acceptance of the revised terms.
